Step 4  Enter the number of badges desired in the appropriate fields.

Step 5  Click here to calculate the total cost.  

Step 6  Print out this page and send it to the following address with the above amount included. MAKE ALL FUNDS PAYBLE TO SCIENCE FICTION FANTASY GUILD. This form must be received by 2/8/08. We will send you a confirmation email once this form has been received
